  • NCS data is organized at a household level. In order to process your request, we require written confirmation from all family members and/or other individuals residing at the address you provided above, confirming they agree that purchase information potentially pertaining to them can be released to you.   • Proof of Frequent Shopper Card Accounts: This can be submitted in the form of photocopies/images of each card or with the original email confirming your registration with the frequent shopper program. *Proof of frequent shopper card ID must show the retailer name and your frequent shopper card ID number.
